Acetabular Reconstruction with Impacted Morselized Cancellous bone Allograft in Revision Total Hip Arthroplasty

Abstract
Purpose: To evaluate the radiographic results of acetabular reconstruction with impacted morselized cancellous bone allograft in revision total hip arthroplasty(THA).

Materials and Methods: Thirty revision THA patients with impacted morselized cancellous bone allograft were analyzed retrospectively. The average patient age was 57 years and the mean follow-up period was 45 months. Radiographic parameters used in this study included bone union, remodeling, rotation center of the hip, migration and angular change of the cup, and radiolucent zone around the cup.

Results: The center of rotation was reconstructed at the anatomical position in 83% of the patients. There were three(10%) radiographic failures, all of which registered combined cavitary/segmental deficiencies preoperatively, and of which re-revision in the near future for symptomatic failure has been decided in only one case. There was also one infection. In all cases except the sole infection, bony union between graft bone and host bone occurred between 6 months and 1 year, followed by remodeling. One case of dislocation and two acases of Brooker grade ¥° heterotopic ossification were observed.

Conclusion: Acetabular bone stock deficiency can be reconstructed successfully using impacted morselized cancellous bone allograft. The radiographic failure of the acetabular component was not always correlated to the clinical failure and the bone reconstruction failure of the acetabulum.
